• Medication Permission Form - please ensure an adult drops and collects all medication at the office. Medication must also be in its original packaging. 
   
• Absence Request Form

Please note, any appointments, e.g. doctors, will require medical evidence to be seen. This can be emailed to the school office or shown in person. Any absence that does not have medical evidence will be marked as an unauthorised absence.
